Health Dynamics Program - Scheduling
To schedule an appointment:
- Call one of the satellite providers listed on the back-side of this handout. All satellite providers provide the same services.
- When you call to schedule your appointment, let the receptionist know that you are with the Operating Engineers Local 49 and that you want to schedule your Health Dynamics exam. This is very important.
- The receptionist will ask for: your name, social security number, address, telephone number, date of birth, the members name and the members social security number. Health Dynamics will check eligibility with the Health Fund, therefore we will need the members name and social security number.
- The receptionist will also ask women if they want the pap smear and ask those women 40 and over if they want the mammogram.
- Once an appointment is scheduled (typically in the morning due to the fasting requirement), the satellite provider will mail an introduction letter (this will include your appointment date and time and instructions for preparing for the exam), a Questionnaire, a Food Record, a map to that particular provider and a breast screen instruction sheet for those women who choose to have the mammogram.
- Please fill out the questionnaire prior to your appointment and bring it with you on the day of your exam.
- When you arrive for you exam you will be taken through the procedures listed above. You can expect your exam to last from 2-3 hours.
- Prior to leaving your initial exam date, you will be scheduled a few weeks later for your consultation session.
- After we receive all of your results, you and your spouse can return together for you consultation session. You and your spouse will each receive an individual report booklet at the personal consultation session. You and one of our health educators or exercise physiologists will review the results and discuss ways in which you can improve your health and fitness levels to achieve better results next year.
- At the time of the consultation session, recommendations may be given for follow-up with primary care physicians (i.e.: elevated PSA level, abnormal EKG, etc.). All follow-up work will be provided through your primary care physician and billed directly to your Health Fund. If you do not have a primary care physician, the satellite provider will be happy to help you find a physician.
- It is important to understand that the information that is obtained through the screening will be retained by Health Dynamics and treated as confidential and will not be released to anyone other than the providers of the Health Dynamics Program or Health Systems Management, a patient advocate group selected by the Operating Engineers Local 49 Health Fund that provides help and information on diagnosis and treatment options if you are confronted with serious health issues. Health Systems Management will contact participants directly when health issues are identified.
